CEU Pre-Approval Form

The ISA-Rocky Mountain Chapter office is now assigning CEUs for functions that take place in our chapter states. If you have a function coming up that is eligible for CEUs please follow these steps:

1. If you are going to offer a function that is eligible for CEUs, please complete the ISA Session/Course Pre-Approval Application for Continuing Education Units.

2. Read the application carefully and fill it out. If your function has several different sessions, you may need a form for each session. In that case, you may copy the form before filling it out as many times as needed. Be sure to include the amount of CEUs for which you are applying.

3. Return the form(s) to the ISA-RMC certification liaison along with a brochure or agenda prepared for your session at least 7 days prior to session. Your function(s) will be assigned a code(s). This will be submitted along with the amount of CEUs assigned. A letter of approval will be sent to you along with attendance/sign-in sheets for class participants. Please allow 2-3 weeks prior to your function for your application to be processed.

4. Is is the instructor’s responsibility to have all participants eligible for CEUs sign the attendance/sign-in sheet(s). When the function is over, the instructor should make copies of the sign-in sheet(s) for his or her records, and return the original copy to the ISA-RMC office. We will keep a copy in our file and forward the original copy to the International Office where they will assign the CEUs to only those listed on the attendance/sign-in sheet(s). Please note: If a participant fails to sign the attendance/sign-in sheet it is the instructor’s responsibility to verify the participant’s attendance.
